How To Fill Out The Service Order Form

ℹ️Telxi requires you to complete a Service Order Form to request SIP trunks and DID numbers. Follow these steps to fill out the form correctly.

1. Company Details

Fill in your company’s information:

  • Company Name – Enter your business’s registered name.

  • VAT Number / Tax ID – Provide your VAT or Tax ID (if applicable).

  • Account Email – Use the email linked to your Telxi account.

⚠️ Note: Ensure your VAT/Tax ID is correct, as it may be required for invoicing.




2. Request Specifications

Answer the following questions:

  1. Do you also need SIP Trunks?

    • Select Yes if you need SIP trunk services.

    • Select No if you only require DID numbers.

  2. SIP Trunk Plan (Only if you selected “Yes” above):

    • Choose between PAYG (Pay As You Go) or Unlimited Plan (€19.99/month).

  3. DID Usage Type:

    • Select the applicable type: Residential, Business, or Toll-Free.

  4. Additional Outbound Concurrent Calls Needed?

    • Select Yes if you need extra outbound calling capacity.

  5. Additional Inbound Concurrent Calls Needed?

    • Select Yes if you need extra inbound calling capacity.


3. DID Ordering Form

Enter your DID (Direct Inward Dialing) number preferences:

  • Select Country – Choose the country for your DID numbers.

  • Area Code – Specify the area code you need.

  • Quantity – Enter how many DID numbers you require.

Additional options:

  • SIP Trunk – Select the SIP Trunk option.

  • Port Existing Number? – Choose Yes if you need to port an existing number.

  • Request Another DID? – Select Yes if you need multiple DIDs.


4. Requested By (Your Information)

Provide your personal details:

  • Full Name – Enter your name.

  • Position – Your role in the company.

  • Phone Number – Contact number for verification.

  • Email – Enter your email address.

  • Website – If applicable, provide your company’s website.

  • Request Date – Select the date of your request.

Signature: Sign electronically or type your name (as per Telxi’s instructions).



5. Accept Terms & Submit

  • Read the Terms & Conditions section.

  • Tick the checkbox to confirm your agreement.

  • Click the Submit button to send your request.


⚠️ Next Steps After Submission

  • Telxi will review your order and may contact you for verification.

  • You will receive confirmation once your request is processed.
